We will not contact you once we receive this form. This form is intended to be printed once completed to include it in the package with your hard drive. This way when we receive your package we know how to contact you, we know what the problem is and what your most important files are to help you.

Once you have completed this form, print the result and include it in your box. A mailing label is provided for your convenience, using this is optional. Submitting a job online is not required, you can simply visit our offices without an appointment to bring a job to us in person as well.

What happens when you use the disk, do you recieve any errors? Do you hear any unusual sounds? If somebody has looked at the disk for you what did they suspect the problem was.*
We require a list of critical files to determine if the data recovery work we complete for you is deemed successfull. Simply typing ‘I need everything’ is not helpful to us, we always try to recover all files if possible. In some cases when we can only temporarily repair a disk, the disk may only work for a short period of time so we need to save the most critical files for you ASAP before the disk fails again. List of most important foldernames and filenames. (e.g. C:\Documents and Settings\User\Mydocuments, desktop Folder, Outlook PST File, C:\Myob18\companyfile.myo)*
